The documents required for a claim

It’s never easy to lose a loved one. It is not pleasant to deal with the paperwork later, especially if you or your family are mourning. It could be a bit simpler if you have access to specific papers and know what to do when the time comes.

Gather the required documents

The following documents are required to claim:

  • Certified copy of main members’ ID.
  • Certified copy of beneficiary’s ID (if deceased is the main member).
  • Certified copy of the deceased member’s ID or birth certificate.
  • Police report in case of unnatural death.
  • Certified copy of the deceased’s death certificate.
  • BI-1663 form with complete information on the notice of death
  • Copy of beneficiary’s bank statement with bank stamp.

Complete the forms

  • In most cases, these are the Claims Form and Beneficiary Form; however, extra paperwork may be needed for fatalities from reasons other than natural causes.

Then submit the forms and documentation to the service provider

  • This can be done electronically (through email) or physically by handing the papers to the service provider.
